Vitreous in the Anterior Chamber is usually caused from trauma. Vitreous is the transparent gel that fills the back of the eye. The lens and the Capsular bag keep the Vitreous from getting to the Anterior Chamber (infront of the lens), this patient had a dislocated IOL which let the vitreous come into the front of the eye.
